We have taken this a step further by exploring ways to help people with disabilities make their own decisions and participate in the community.

Using a person-centered planning process that involves the individual, family members and others who know and care about the person, we gather information on the individual's positive attributes and capacities and develop a personal profile that shapes a vision for the future. The resulting action plan outlines the steps that will help each person achieve their personal goals.

Our staff also helps individuals create the relationships in their communities that make their lives meaningful. Because each person's needs differ, we work with individual participants and their families to help secure appropriate training and support.
